Job summary

CareSource is seeking an Integrated Care Clinical Manager in Massachusetts. The RN will monitor and manage care for dually-eligible enrollees with complex needs, coordinating with physicians and community resources to improve health outcomes.

The role involves leading the ICT, performing comprehensive assessments, and guiding care plans while ensuring access to long-term services and preventive care.

Qualifications

• Associates of Science degree in Nursing from an accredited program.
• Registered Nurse with ability to independently serve people with complex medical, behavioral and social needs.
• Prior experience in care coordination or case management with dual-eligible populations is preferred.

Responsibilities

• Engage enrollees in their homes and community settings to build a complex care management relationship.
• Coordinate with healthcare providers and community resources to ensure seamless care transitions.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Oversee enrollees' utilization of long-term services to keep them in their chosen location.
• Assist members in accessing housing, transportation, food assistance, and social services.
• Educate members about benefits, services, and preventive care.
