Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager (RN) to monitor and manage complex dual-eligible enrollees. You will coordinate medical, behavioral, and social services, working to improve health outcomes and quality of life. This mobile role requires travel to member homes and community sites.

You will lead an interdisciplinary team, develop care plans, educate members on benefits, and sustain coordinated care across providers and resources to prevent hospital readmissions.

Qualifications

• Associates of Science in nursing from an accredited program is required.
• Current unrestricted RN license in Massachusetts is required.
• Experience in care coordination or dual-eligible populations preferred.
• Medicare/Medicaid knowledge and community resources understanding.

Responsibilities

• Engage enrollees in homes and community settings.
• Lead interdisciplinary care team to develop holistic care plans.
• Coordinate with providers and community resources for care transitions.
• Educate members about benefits, services, and preventive care.
