Job summary

Commonwealth Care Alliance is seeking an Integrated Care Clinical Manager in Massachusetts. The RN will monitor, follow up, and manage care for dually-eligible enrollees with complex medical and social needs.

Qualifications

• Associates of Science degree in nursing from an accredited program.
• A Registered Nurse with the ability to independently serve people with complex needs.
• Case management experience preferred for dual-eligible populations.

Responsibilities

• Engage with enrollees in homes and community settings to establish complex care management relationships.
• Coordinate with healthcare providers and community resources to ensure seamless care transitions.
• Lead the interdisciplinary care team to develop holistic care plans addressing medical and non-medical needs.
