Job summary

Monogram Health is seeking a Care Manager – Registered Nurse to join an integrated care team providing in-home care to CKD/ESRD patients. You will develop and adapt care plans, perform visits, monitor vitals, and educate patients and families to improve quality of life.

This role requires BLS, an active RN license in the posting state, and 2+ years in care management with travel to patient homes. Monogram Health emphasizes compassionate, coordinated care across disciplines.

Qualifications

• Licensed RN in the state of the posted location.
• 2+ years experience in care management and/or with CKD/ESRD patients.
• Ability to travel locally for in-home visits.

Responsibilities

• Develop and continually adapt care plans with patients' medical providers.
• Perform in-home care management visits and monitor biometrics.
• Educate patients and families on CKD/ESRD, treatment options, and lifestyle.
• Coordinate with pharmacists, prescribers, and dialysis providers.
• Document progress in the care management platform and support transitions of care.
