Job summary

Monogram Health Inc is seeking a dedicated Registered Nurse to join the care management team. You will work within a small patient panel, developing and updating individualized care plans in collaboration with physicians and a social worker.

Your role includes in-home visits, assessments, and education to improve quality of life for CKD/ESRD patients. The position offers a hybrid in-home model with local travel and competitive compensation, plus a full benefits package and a performance-based

Qualifications

• Graduate of an accredited School of Nursing.
• Current RN license in the posting state.
• 2+ years in care management and/or CKD/ESRD care.
• Willingness to travel locally for in-home visits.
• BLS certification provided/onboarded by employer.

Responsibilities

• Develop and adapt care plans with patients’ medical teams.
• Conduct in-home care management visits to implement care plans.
• Monitor biometric data and intervene per approved protocols.
• Coordinate medications with pharmacists and prescribers.
• Conduct patient health assessments and surveys as needed.
• Educate patients on CKD/ESRD, dialysis, and comorbidities.
