Job summary

Monogram Health is seeking a Care Manager RN to work in patients’ homes, collaborating with a care team to manage CKD/ESRD. You will build trusting relationships, coordinate with providers, and deliver education on medications, diet, and lifestyle choices. This role involves travel and requires a current RN license and BLS certification.

Qualifications

• Demonstrates empathy and strong communication with patients and families.
• Ability to work independently with minimal supervision.
• Familiarity with Microsoft Office and mobile/web-based tools.

Responsibilities

• Develop and continually adapt care plans with patients’ medical providers.
• Perform in-home care management visits to execute care plans.
• Monitor biometric data and follow protocols for interventions.
• Reconcile medications and coordinate with pharmacists and prescribers.
• Educate patients on CKD/ESRD, dialysis, and related comorbidities.
• Ensure adherence to treatments and encourage proactive care decisions.
