Job summary

Monogram Health is seeking a Registered Nurse Care Manager to lead in-home CKD/ESRD patient care. You will partner with physicians, social workers, and interdisciplinary teams to develop and adjust care plans while ensuring quality outcomes.

Qualifications

• Active RN license in the state of the posting.
• 2+ years in care management and/or CKD/ESRD care.
• BLS certification required; company supports onboarding.
• Able to travel locally for in-home visits.
• Excellent verbal communication skills.

Responsibilities

• Develop and adapt care plans with patients' medical providers.
• Perform in-home visits to execute care plans.
• Monitor biometric data and follow protocols.
• Coordinate with pharmacists and prescribers.
• Educate patients on CKD/ESRD, dialysis and comorbidities.
• Engage families and social support groups in care.
• Serve as primary contact during business hours.
• Educate on medications, conditions, diet, exercise, and lifestyle.
