Job summary

Monogram Health is seeking a Registered Nurse to join our Care Manager team. The RN will work closely with a physician-led care team to manage in-home care for patients with CKD/ESRD, aiming to improve quality of life and slow disease progression.

Responsibilities include developing and updating care plans, performing in-home visits, educating patients and families, coordinating with pharmacists and dialysis providers, and documenting progress in the care management platform.

Qualifications

• Graduate of an accredited School of Nursing.
• Currently licensed as an RN in the posting state.
• 2+ years in care management or CKD/ESRD care.
• Willingness to travel locally for in-home visits.

Responsibilities

• Develop and adapt care plans with patients and providers.
• Perform in-home care management visits and monitor biometric data.
• Educate patients and families on CKD, ESRD, dialysis, and comorbidities.
• Coordinate with dialysis providers to ensure smooth transitions of care.
• Document patient updates in the care management system.
