Job summary

Monogram Health is seeking a Care Manager RN to provide in-home care for patients with chronic kidney disease and related conditions. The role involves partnering with physicians, nurses, and social workers to develop and implement personalized care plans.

The RN will conduct in-home visits, monitor patient health, and educate patients and families on treatments, lifestyle, and medications to improve quality of life and slow disease progression.

Qualifications

• RN licensure in the state of posting location.
• Graduate of an accredited School of Nursing.
• 2+ years in care management and/or CKD/ESRD care.

Responsibilities

• Develop and continually adapt patient care plans with medical providers.
• Perform in-home care management visits.
• Monitor biometric data and adhere to protocols.
• Coordinate medications with pharmacists and prescribers.
• Conduct health assessments and patient surveys.
• Educate patients on CKD, ESRD, dialysis and comorbidities.
