Job summary

Monogram Health seeks a Care Manager RN to lead in-home care plans, partnering with medical providers to enhance quality of life for patients with CKD/ESRD. You will manage visits, monitor health data, and educate patients and families while coordinating with pharmacists and care teams.

This role requires local travel, RN licensure in the assigned state, and 2+ years in care management. Comprehensive benefits and 401k with employer match are provided.

Qualifications

• Graduate of an accredited School of Nursing.
• Licensed RN in the posting location state.
• 2+ years in care management or CKD/ESRD settings.

Responsibilities

• Develop and continually adapt care plans with patients and medical providers.
• Perform in-home care management visits to execute care plans.
• Monitor biometric data and follow protocols for interventions.
• Coordinate with pharmacists and prescribers on medications.
• Educate patients and families on CKD, ESRD, dialysis, and comorbidities.
• Manage transitions of care during hospital discharges.
