Job summary

Monogram Health seeks a Care Manager RN to join a multispecialty team delivering in-home, evidence-based CKD/ESRD care. You will work with an Advanced Practice Provider and a Social Worker to improve patients' quality of life at home and slow disease progression.

You will perform in-home visits, monitor vitals, educate patients and families, coordinate with dialysis providers, and support proactive care decisions. Local travel is required.

Qualifications

• Active RN license in the state of posting location.
• 2+ years of care management and/or CKD/ESRD experience.
• BLS certification required; employer supports onboarding.
• Willingness to travel locally for in-home visits.

Responsibilities

• Develop and adapt care plans with patients' medical providers.
• Perform in-home care management visits to execute plans.
• Monitor biometric data and follow protocols for interventions.
• Coordinate medications with pharmacists and prescribers.
• Educate patients and families about CKD/ESRD and lifestyle.
• Engage families and social supports in education and care.
