Job summary

Your Health is seeking a skilled Primary Care Physical Therapy Assistant to join our growing team. You will work with licensed physical therapists to provide rehabilitative services to homebound patients, conducting exercises, educating patients, and documenting progress during visits to homes, ALFs, and ILFs.

The position is full-time and salary-based with a monthly bonus. Two schedules are available, Monday–Friday 8:00 AM–5:00 PM, serving the Brunswick area with compassionate, patient-centered

Qualifications

• Associate degree from an accredited PTA program.
• Current licensure or eligibility to practice as a PTA.
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work in a multidisciplinary team.
• Excellent organizational and time management skills.
• Compassion and empathy for rehabilitation patients.
• Physical stamina and dexterity for mobility and therapy activities.
• Pelvic health experience or interest.

Responsibilities

• Assist in implementing treatment plans with licensed therapists.
• Conduct therapeutic exercises with patients and monitor progress.
• Educate patients on proper rehabilitation techniques.
• Document patient progress and update records accurately.
• Develop individualized treatment plans and adjust as needed.
• Instruct patients in therapeutic exercises and activities.
• Maintain a clean, organized therapy environment and sanitize equipment.
• Coordinate discharge plans and follow-up care with the team.
