Job summary

Your Health Organization is seeking a skilled Physical Therapy Assistant to support licensed therapists in delivering rehabilitative care to homebound patients across our service areas. You will help implement treatment plans, guide patients through exercises, document progress, and assist with discharge planning.

Two schedules are available: Monday–Friday 8:00 AM–5:00 PM with one weekend per month, or Monday–Sunday 7:00 AM–7:00 PM (7 days on, 7 days off).

Qualifications

• Associate degree from an accredited PTA program is required.
• Current licensure or eligibility to practice as PTA in the state.
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work in a multidisciplinary team.
• Excellent organizational and time management skills.

Responsibilities

• Assist licensed physical therapists in implementing treatment plans.
• Guide patients through therapeutic exercises and activities.
• Document patient progress and adjust plans as needed.
• Assist in discharge planning and provide home care instructions.
