Job summary

Your Health Organization is seeking a skilled and motivated Primary Care Physical Therapy Assistant to join our growing primary care family. You will work closely with licensed physical therapists to provide rehabilitative services to homebound patients, helping individuals recover from injuries or illnesses.

This is a full-time, salary-based position with two schedules available including occasional weekends.

Qualifications

• Associate degree from an accredited Physical Therapy Assistant program.
• Current licensure or eligibility for licensure as a PTA in the state of practice.
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work in a multidisciplinary team.
• Excellent organizational and time management skills.
• Compassion and empathy for patients undergoing rehabilitation.
• Pelvic health experience and/or interest.

Responsibilities

• Assist in implementing treatment plans with licensed therapists.
• Conduct exercises with patients and educate them on techniques.
• Document patient progress and develop individualized plans.
• Instruct and assist patients in performing therapeutic activities.
• Monitor patients during therapy sessions and adjust plans as needed.
• Maintain a clean, organized therapy environment and sanitize equipment.
• Coordinate discharge plans and follow-up care with care teams.
