Job summary

Your Health Organization is seeking a skilled Physical Therapy Assistant to join our growing primary care team. You will work closely with licensed physical therapists to deliver rehabilitative services to homebound patients, assisting with exercise instruction, treatment implementation, and progress documentation.

The role involves home, ALF's, and ILF's visits, with a salary-based compensation and potential monthly bonus.

Qualifications

• Associate degree from an accredited PTA program.
• Current licensure or eligibility for licensure in the state of practice.
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work effectively as part of a multidisciplinary team.
• Excellent organizational and time management skills.
• Compassion and empathy for patients undergoing rehabilitation.
• Physical stamina and dexterity to assist patients with mobility.

Responsibilities

• Assist licensed physical therapists in implementing treatment plans.
• Conduct therapeutic exercises with patients and educate on proper techniques.
• Document patient progress and adjust plans as needed.
• Develop individualized treatment plans and instruct patients on exercises.
• Monitor patients during therapy sessions and maintain a clean, organized therapy environment.
• Coordinate discharge plans and follow-up services with care teams.
