Your Health is hiring a Primary Care Physical Therapy Assistant to join our growing team. You will support licensed physical therapists in delivering rehabilitative services to homebound patients across Savannah, GA, and surrounding areas.

The role involves conveying treatment plans, guiding patients through exercises, documenting progress, and coordinating with the care team. We offer competitive compensation with monthly bonus opportunities and a strong benefits package.

Qualifications

• Associate degree from an accredited PTA program.
• Current PTA licensure or eligibility in the practicing state.
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work effectively in a multidisciplinary team.
• Compassionate, empathetic approach to patient care.
• Physical stamina to assist patients with mobility.
• Pelvic health experience or interest.

Responsibilities

• Assist licensed PTs in implementing treatment plans.
• Conduct therapeutic exercises with patients.
• Educate patients on proper rehab techniques.
• Document patient progress accurately.
• Develop individualized treatment plans and monitor progress.
• Maintain a clean, organized therapy environment and sanitize equipment.
• Coordinate discharge plans with the care team.
