Job summary

The Registered Nurse-Home Based Primary Care (HBPC) Outpatient Staff provides competent, evidence-based care to HBPC patients and supports care coordination for Veterans within the HBPC program.

This role involves oversight of licensed vocational nurses and nursing assistants as appropriate, direct patient care, and collaboration with interdisciplinary teams to optimize Veteran outcomes.

Qualifications

• English proficiency in speaking and writing is required.
• Graduate of a school of professional nursing approved by accrediting bodies (ACEN/CCNE) or equivalent.
• Current, full, active, and unrestricted Registered Nurse license in a US state, territory, or DC.

Responsibilities

• Provide competent, evidence-based direct patient care to HBPC Veterans.
