Job summary

U.S. Department of Veterans Affairs is seeking a Nurse for Home Based Primary Care (HBPC) to provide direct Veteran care and care coordination in the home setting.

The HBPC RN Case Manager delivers comprehensive, interdisciplinary primary care, conducts initial and ongoing assessments, teaches Veterans and caregivers, and ensures care coordination across teams. Strong English proficiency and an active RN license are required.

Qualifications

• English language proficiency required.
• Graduate of a school of professional nursing approved by accrediting bodies.
• Current, full, active, and unrestricted RN license in a US state or territory.
• Foreign education considered with proper validation.

Responsibilities

• Provide initial and continued assessment of nursing needs for Veterans.
• Teach Veterans and Caregivers effective home-care management.
• Monitor Veterans’ physical and psychosocial status.
• Oversee the provision of direct care and coordinate with HBPC team.
• Adhere to visit schedules (monthly visits for first 90 days, then at least every 60 days).
