Job summary

The HBPC RN Case Manager provides comprehensive home-based primary care to Veterans with complex medical, social, and behavioral needs. The role emphasizes interdisciplinary coordination, patient and caregiver education, and ongoing assessment within the HBPC program.

Qualifications

• English language proficiency is required.
• Graduate of an approved nursing program with RN licensure.
• Possession of State license to practice nursing.
• Foreign nursing education requires credential evaluation when applicable.

Responsibilities

• Provide initial and ongoing assessment of nursing needs.
• Teach veterans and caregivers how to manage health at home.
• Monitor patient condition and care at home.
• Oversee direct patient care and delegate tasks as needed.
• Visit veterans monthly for the first 90 days, then at least monthly.
