Job summary

The Nurse Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) executes position responsibilities that demonstrate leadership - experience - and creative approaches to management of complex HBPC care. This nurse provides direct Veteran care and care coordination - as it pertains - to Veterans and their care within the HBPC Program.

Duties include leading nursing processes, coordinating care at home, and ensuring high-quality outcomes for Veterans served by HBPC.

Qualifications

• English Language Proficiency in written and spoken English.
• Graduate of a school of professional nursing approved by the appropriate accreditation body.
• Current, full, active and unrestricted nursing license in a US state or territory.
• Grandfathering provision applies for VA HBPC positions existing at the standard date.

Responsibilities

• Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
• Plans and delivers technically competent care in the home of Veterans with complex needs.
• Utilizes interventions to maximize health capabilities and reduces unnecessary urgent care and emergency visits.
• Makes home visits to Veterans in the program.
