Job summary

Veterans Health Administration, Department of Veterans Affairs in Miami seeks an experienced RN for Home Based Primary Care Outpatient. This role provides direct patient care in patients' homes with coordination across a multidisciplinary team and leadership in HBPC initiatives.

The HBPC RN delivers evidence-based care, participates in education, and supports quality improvement and outcomes utilization within a VA academic hospital program.

Qualifications

• Must be a graduate of an approved nursing program and eligible for RN licensure.
• Must meet VA requirements including English proficiency and licensure.
• Foreign nursing education requires degree equivalency validation.
• Must pass pre-employment physical as required by VA Handbook 5019

Responsibilities

• Provide competent, evidence-based Home Based Primary Care to Veterans with oversight of other staff as appropriate.
• Lead case management: assessment, triage, education, resource coordination, and care planning.
• Make home visits and coordinate transitions across the care continuum.
• Collaborate with interdisciplinary team to promote safe transitions and quality outcomes.
• Direct patient care and care coordination within the HBPC program.
