Job summary

The Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) provides leadership, experience, and creative management of complex HBPC care for Veterans at the Joplin CBOC. The role involves direct patient care, care coordination, and education to support Veterans in their homes and communities.

Qualifications include an ACEN/CCNE-accredited nursing program, RN licensure in good standing, and English proficiency.

Qualifications

• English language proficiency is required for direct patient care.
• Graduate of an ACEN or CCNE-accredited nursing program.
• Current, full, active, and unrestricted RN license in a U.S. state or territory.
• For foreign-trained nurses, degree equivalency validation may be required.
• Bridge program graduates may pursue licensure prior to completing bridge program.

Responsibilities

• RN duties include case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
• Plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion and palliative care.
• Utilizes interventions to maximize health and reduce urgent care visits according to Veterans' goals of care.
• Makes home visits to Veterans in the program.
• Collaborates to develop, implement, evaluate, and revise HBPC policies and procedures.
• Develops and coordinates individual care plans with ongoing assessment and re‑assessment, care coordination, monitoring, and evaluating outcomes.
• Coordinates transitions of care across settings through communication with the interdisciplinary team.
• Provides education and support to patients and caregivers to optimize outcomes and quality of life.
• Delegates and supervises care by LPNs and home health technicians as applicable.
• Performs intensive care management for assigned patients and links Veterans with resources.
• Documents assessments and plans in HBPC SOP and ensures safe medication administration.
