Job summary

U.S. Department of Veterans Affairs is seeking a Home-Based Primary Care (HBPC) Registered Nurse (RN) to plan and deliver care in Veterans' homes with complex needs.

This role focuses on health maintenance, care coordination, and improving quality of life for Veterans in a community setting. The HBPC RN will conduct comprehensive care planning, coordinate with interdisciplinary teams, and ensure adherence to VA care standards.

Qualifications

• English language proficiency required.
• Current, full, active nursing license in a US state or territory.
• Graduate of a nursing program approved by a recognized accrediting body (ACEN/CCNE).
• Bridge program path: master’s level with nursing coursework may lead to RN licensure.
• Equivalency validation required for foreign-educated nurses.

Responsibilities

• Provide direct Veteran care and care coordination within the HBPC program.
• Conduct case management, assessment, triage, veteran education, resource coordination, medication and treatment coordination.
• Plan and deliver competent home-based care for Veterans with complex needs, including health promotion and palliative care.
• Utilize interventions to maximize health and reduce unnecessary urgent care visits.
• Make home visits to Veterans enrolled in HBPC.
• Collaborate on policies, procedures and guidelines, and evaluate outcomes of care.
