Bloom Healthcare is seeking a Primary Care NP/PA to deliver patient-centered, home- and community-based primary care in Centennial, CO. You will work with an interdisciplinary team to manage complex chronic conditions and respond to acute needs, supporting seniors where they live.

Qualifications

• NP or PA with master’s degree and active Colorado licensure.
• Experience serving seniors or home-based patients is preferred.
• Mission-driven, patient-centered, relationship-focused care is essential.

Responsibilities

• Provide longitudinal, home- and community-based primary care.
• Diagnose and manage chronic and acute conditions with support from Bloom’s team.
• Address social determinants of health and coordinate post-discharge care within 72 hours.
• Collaborate with MD leadership, triage, schedulers, IT, and care teams to ensure continuity.
• Conduct comprehensive evaluations, meds reconciliation, and health education for patients and caregivers.

Education

Master’s degree in Nursing or Physician Assistant studies
