Job summary

Bloom Healthcare in Loveland, CO is seeking a Primary Care NP or PA to deliver patient-centered, home- and community-based primary care.

You will care for a defined patient panel, diagnose and manage chronic and acute conditions, and collaborate with an interdisciplinary team supported by advanced data systems.

Qualifications

• NP or PA with master's degree and active Colorado licensure.
• Commitment to serving seniors and medically complex populations.
• Strong communication and home-based care adaptability.

Responsibilities

• Deliver longitudinal, holistic medical care in patient homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with team support.
• Address social determinants of health and access barriers.
• Conduct specialty visits such as wound care and post-hospital transitional care.
• Complete comprehensive evaluations including medication reconciliation and health education.
• Coordinate post-discharge visits within 72 hours and track outcomes with data systems.
• Collaborate with internal/external care teams to reduce readmissions.
