Job summary

Bloom Healthcare is seeking an NP/PA to deliver primary care in homes and communities, with autonomy and a supportive interdisciplinary team. You will manage complex patients, coordinate care, and address social determinants of health to improve outcomes.

Qualifications

• Master’s degree or higher and active Colorado licensure.
• Experience or aptitude for home-based primary care.
• Strong communication and relationship-building skills.
• Commitment to patient-centered, autonomous practice in patients’ homes.

Responsibilities

• Serve as the primary care provider for a defined patient panel in homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with team support.
• Address social determinants of health and access barriers.
• Conduct wound care, post-hospital transitional care, and condition-specific assessments.
• Complete comprehensive evaluations including med reconciliation and fall risk assessments.
• Track hospitalizations and coordinate timely post-discharge visits (within 72 hours).
• Collaborate with internal and external care teams to reduce readmissions and stabilize high-risk patients.
