Job summary

Bloom Healthcare is seeking a Primary Care NP/PA to deliver high-quality, home- and community-based primary care in Golden, CO. You’ll lead longitudinal care for a defined patient panel with independence and strong interdisciplinary support.

Qualifications

• NP or PA with a Master’s degree and active Colorado licensure.
• Experience in primary care or home-based settings is a plus.

Responsibilities

• Provide relationship-based primary care in patients' homes and communities.
• Diagnose and manage chronic and acute conditions with MD leadership support.
• Address social determinants of health affecting housing, nutrition, and caregiver support.
• Conduct wound care, post-hospital transitional visits, and condition-specific assessments.
• Perform comprehensive evaluations including med reconciliation and health education.
