Job summary

Bloom Healthcare is seeking a Nurse Practitioner or Physician Assistant for primary care delivered in patients' homes and communities in Loveland, CO. You’ll provide longitudinal, relationship-based care with autonomy and the support of an interdisciplinary team.

The ideal candidate holds a master’s degree and Colorado licensure, enjoys complex primary care, and thrives in home-based environments that value patient-centered outcomes.

Qualifications

• Licensed NP or PA with master’s degree and Colorado licensure.
• Ability to manage independent patient panels in home and community settings.
• Strong communication and relationship-based care for seniors.

Responsibilities

• Be primary care provider for defined patient panel, in homes and communities.
• Diagnose, manage, and treat chronic/acute conditions with team support.
• Address social determinants of health and access barriers.
• Conduct wound care, post‑hospital follow-ups, and condition-specific assessments.
• Perform comprehensive evaluations including med reconciliation and fall risk assessments.
• Utilize integrated data to track hospitalizations and coordinate post-discharge visits.
• Collaborate with internal/external teams to ensure continuity of care.
