Job summary

Bloom Healthcare, an award-winning, employee-owned medical practice, seeks a Primary Care NP/PA to deliver high-touch primary care at home and in communities around Parker, CO. You will partner with an interdisciplinary team to manage complex patients with autonomy and a mission-driven culture.

You’ll provide relationship-based primary care, respond to acute needs, and drive outcomes using Bloom’s data systems. Excellent clinicians thrive here.

Qualifications

• Must hold an active Colorado NP or PA license.
• Master’s degree required.
• Experience delivering home-based primary care is preferred.

Responsibilities

• Provide longitudinal primary care in patients' homes and communities.
• Diagnose and manage chronic/acute conditions with team support.
• Address social determinants of health and care coordination.
• Perform wound care, post-hospital transitions, and assessments.
• Ensure comprehensive evaluations including medication reconciliation and health education.
• Utilize data systems to track hospitalizations and coordinate post-discharge visits (within 72 hours).
• Collaborate with care teams to reduce readmissions and stabilize high-risk patients.
