Job summary

Bloom Healthcare in Centennial, CO seeks a Primary Care NP/PA to deliver relationship-based, home-based primary care to medically complex seniors. You will work with an interdisciplinary team and use advanced data systems to improve health outcomes.

You’ll practice independently with strong support from MD leadership and care coordinators while maintaining a manageable patient panel and a patient-centered approach.

Qualifications

• Active Colorado license required.
• Experience in home-based or community primary care preferred.
• Comfort practicing independently with team support.

Responsibilities

• Provide longitudinal, relationship-based primary care in patients' homes and communities.
• Diagnose and manage chronic and acute conditions with interdisciplinary support.
• Address social determinants of health and coordinate care with MD leadership.
• Conduct preventive care, wound care, and post-discharge follow-ups as needed.
• Track patient outcomes using Bloom’s data systems and ensure timely visits.
