Job summary

Bloom Healthcare in Thornton, CO is seeking a Nurse Practitioner or Physician Assistant to deliver relationship-based primary care in patients’ homes and local communities. This autonomous role emphasizes high-quality, patient-centered care.

You’ll diagnose and manage chronic and acute conditions with support from an interdisciplinary team, address social determinants of health, coordinate timely visits, and use Bloom’s data systems to improve outcomes while maintaining a sustainable workload.

Qualifications

• NP or PA with a master's degree and active Colorado licensure.
• Experience delivering home-based primary care to seniors.
• Strong communication and relationship-building skills.

Responsibilities

• Provide longitudinal primary care in patients' homes and communities.
• Diagnose and manage chronic and acute conditions with team support.
• Address social determinants of health, including housing and caregiver needs.
• Conduct post-visit follow-ups and health education for patients and caregivers.
• Coordinate care using Bloom’s data systems to track outcomes.
• Collaborate with internal and external care teams to reduce readmissions.
