Job summary

Bloom Healthcare in Greeley, CO is seeking a Primary Care NP/PA to deliver relationship-based care in patients' homes and communities, supported by an interdisciplinary team and advanced data systems.

You will manage chronic and acute conditions, address social determinants of health, and coordinate post-discharge visits while maintaining a patient-centered, unrushed practice. Compensation ranges from $100k to $125k with strong benefits.

Qualifications

• Master’s degree required and active Colorado license.
• Experience delivering primary care in home/community settings.
• Comfort with independent practice and interdisciplinary teamwork.

Responsibilities

• Provide longitudinal primary care in patients' homes and communities.
• Diagnose and manage acute and chronic conditions with team support.
• Address social determinants of health and caregiver needs.
• Conduct routine assessments, medication reconciliation, and care planning.
• Coordinate post-discharge follow-ups and utilize data systems to monitor outcomes.
• Collaborate with care teams to ensure continuity and reduce readmissions.
