Job summary

THERAPY 2000 is seeking a licensed Speech-Language Pathologist to provide home-based pediatric therapy across the Texas area. You will evaluate and treat children, develop and monitor home exercise programs, and coordinate care with a multidisciplinary team.

This role emphasizes collaboration, innovation, and compassionate patient care. Candidates should hold a TX SLP license and show reliability and adaptability to shifting priorities in a home-health setting.

• Active TX SLP license required.

Responsibilities

• Perform comprehensive Speech Therapy evaluations using standardized tests.
• Develop and implement treatment plans per physician orders.
• Create home exercise programs with caregivers.
• Coordinate care with other providers in the patient’s medical team.
• Complete minimum weekly patient visits as required by the agency.
• Maintain timely chart updates and documentation.
• Attend meetings to align expectations and standards.
• Comply with all agency policies, ethics, and laws for home-health pediatrics.
