Responsibilities

• Perform comprehensive speech therapy evaluations using standardized tests.
• Develop and implement treatment plans per physician orders.
• Create home exercise programs with caregivers.
• Collaborate across providers to coordinate care.
• Meet min patient visit requirements weekly and complete charting timely.
• Adhere to agency policies and home-health regulations.
