Job summary

Empatia is seeking a compassionate Palliative Care Nurse Practitioner to deliver advanced palliative care in patients' homes, collaborating with the Palliative Medical Director to guide families through treatment and improve quality of life.

You will assess, plan, and implement individualized care, maintain documentation, and participate in evening/weekend call rotations. Required MSN/NP licensure and 2+ years of NP experience.

Qualifications

• MSN from an accredited nursing program and NP licensure.
• Minimum two years as a Nurse Practitioner.
• Family or Adult Medicine specialization preferred.
• Part B billing experience is a plus.
• Collaborative agreement with supervising physician or willingness to establish.

Responsibilities

• Collaborate with medical director to deliver patient-centered palliative care in home settings.
• Conduct in-depth assessments including physical, emotional, and psychosocial factors.
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans based on patient progress.
• Prescribe medications and order tests as permitted by state law.
• Maintain compliant clinical documentation and communication with the care team.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals to additional care services when appropriate.
