Job summary

Empatia is seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our interdisciplinary home health team. You will provide advanced palliative care to patients in their homes, collaborating with the Palliative Medical Director to optimize quality of life and comfort.

You will conduct comprehensive assessments, develop care plans, manage treatment adjustments, prescribe medications per state law, and document to professional standards.

Qualifications

• Master of Science in Nursing from an accredited school of nursing (or equivalent per state law).
• Minimum two years’ Nurse Practitioner experience; family or adult medicine preferred.
• Licensure as a Nurse Practitioner with RN license and state requirements.

Responsibilities

• Collaborate with the Palliative Medical Director to deliver comprehensive home-based palliative care.
• Perform in-depth patient assessments including physical, emotional, and psychosocial evaluations.
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans based on patient progress.
• Prescribe medications (including controlled substances) and order tests as permitted by state law.
