Job summary

Gentiva seeks a compassionate Palliative Care Nurse Practitioner to join our interdisciplinary team in delivering high-quality home-based palliative care. You will assess, plan, and manage complex cases, collaborating with physicians and caregivers to optimize quality of life.

Must hold an NP with active RN license, be able to work autonomously, and participate in after-hours rotation. Experience with Part B billing and palliative care preferred.

Qualifications

• Requires MSN-educated NP with state licensure and collaborative agreement.
• Minimum two years NP experience in palliative or relevant field.
• Experience with Part B billing desirable.
• Ability to work autonomously in advanced practice role.

Responsibilities

• Deliver advanced palliative care to home-based patients.
• Perform comprehensive assessments (physical, emotional, psychosocial).
• Develop and adjust individualized care plans with patients/families.
• Prescribe medications and order tests as permitted by state law.
• Maintain compliant clinical documentation per standards.
• Collaborate with care team and physicians.
• Participate in evening/weekend on-call rotation.
