Job summary

Empatia seeks a compassionate Palliative Care Nurse Practitioner to provide advanced in-home palliative care, collaborating with the Palliative Medical Director to optimize quality of life for patients with complex needs.

You will independently assess, plan, and adjust care, prescribe medications, and coordinate with families and the care team, including after-hours calls as required.

Qualifications

• MSN degree from an accredited program.
• Two years NP experience minimum.
• Family or Adult Medicine NP specialty preferred.
• Part B Provider billing experience.
• Ability to work autonomously in advanced practice.
• Current RN license and NP certification/licensure per state law.
• Collaborative agreement with supervising physician (or willingness to establish).
• DEA license (Federal and state as required).
• ACHPN certification in Hospice and Palliative Care preferred.
• Valid driver’s license and auto insurance.

Responsibilities

• Deliver comprehensive, patient-centered palliative care with the care team.
• Perform in-depth patient assessments (physical, emotional, psychosocial).
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans as needed.
• Prescribe medications and order diagnostics per state law.
• Maintain timely clinical documentation per standards.
• Communicate with physicians, caregivers, and other professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional services when appropriate.
