Job summary

Providence at Home with Compassus is seeking a full-time Palliative Care Nurse Practitioner to lead the community-based palliative program for adults. The NP will conduct in-person home visits with oversight from the medical director and program director, coordinating with PCPs and specialists.

A MSW and nurse team member collaborate closely with the NP and program leadership. The role involves a Monday–Friday schedule (8a–5p local), a census load of around 80 patients, and regional travel.

Qualifications

• Master's degree in nursing is required.
• Active RN license in state of practice is required.
• National Provider BLS certification required upon hire.

Responsibilities

• Perform routine and urgent assessments.
• Prescribe medications including controlled substances within scope.
• Collaborate with Medical Director, attending physicians, hospital staff, and IDT.
• Complete and submit clinical documentation within 24 hours of visit.
