Job summary

Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to provide advanced home-based palliative care, helping patients achieve the best possible quality of life while managing complex needs. You will work autonomously with the Palliative Medical Director, guiding patients and families through treatment decisions.

You will perform in-depth assessments, develop individualized plans, adjust treatments, and communicate with the care team.

Qualifications

• MSN from an accredited program and NP licensure as required by state law.
• Minimum two years of Nurse Practitioner experience in a relevant field.
• Family or Adult medicine specialization preferred.
• Experience with Part B billing is a plus.
• Ability to work autonomously in an advanced practice role.

Responsibilities

• Collaborate with the Palliative Medical Director to deliver patient-centered care.
• Perform comprehensive assessments including physical, emotional, and psychosocial factors.
• Develop and implement individualized care plans with patients and families.
• Adjust treatment plans as patient needs change.
• Prescribe medications (including controlled substances) and order tests as allowed.
• Maintain accurate clinical documentation per standards.
• Communicate with physicians, caregivers, and other health professionals.
• Participate in evening/weekend call rotation as needed.
• Refer patients to additional care services when appropriate.
