Job summary

Compassus seeks a Nurse Practitioner for palliative and hospice care across multiple patient locations. You will perform routine and emergency assessments, manage symptoms, and educate patients and families while coordinating with the care team.

Master’s in Nursing is required, NP license in the state, and active NPI; board certification in Hospice/Palliative Care is preferred. Willingness to travel and adapt to scheduling needs is essential.

Qualifications

• Master’s degree in Nursing required.
• Active CNS or Nurse Practitioner license in the state of employment required.
• Board Certification in Hospice and Palliative Care preferred.
• Active NPI number required.
• Active DEA certification if state allows prescription of controlled substances.

Responsibilities

• Performs routine and emergency palliative care assessments and documentation.
• Provides patient and family education on disease and symptom management.
• Collaborates with Medical Director, Attending Physician, DCS and IDT to coordinate care.
• Prescribes medications including controlled substances as delegated and licensed.
• Maintains confidentiality and adheres to CMS guidelines.
