Job summary

You will collaborate with patients, families, and an interdisciplinary team to develop individualized plans aligned with physician orders and agency policies.

Qualifications

• Current license to practice occupational therapy in the state you are assigned to work.
• Graduate of an ACOTE-accredited OT program.
• Eligible to take or has completed the NBCOT entry‑level certification exam.
• One year related occupational therapy experience.
• If less than one year experience, regional clinical leaders' approval and mentoring/residency program may be required.
• Current CPR certification.

Responsibilities

• Responsible for all aspects of occupational therapy service delivery under a physician approved plan of care.
• Ensures the plan of care addresses all problems identified in the assessment or documents rationale if not addressed.
• Accountable for the safety and effectiveness of the occupational therapy service delivery process.
• Completes documentation timely and accurately.
• Supervises home health aide when RN is not active in the case.
• Other duties as assigned.
