Qualifications

• Current license to practice occupational therapy in the practicing state.
• Graduate of ACOTE-accredited OT program.
• Eligible for or passed NBCOT certification.
• Minimum one year OT experience; regional leadership approval if less than one year.
• Current CPR certification.

Responsibilities

• Deliver OT services under a physician-approved plan of care.
• Ensure plan of care addresses identified problems and documents rationale.
• Maintain safety and effectiveness of OT service delivery.
• Complete timely and accurate documentation.
• Supervise home health aide when RN is not active in the case.
• Perform other duties as assigned.
