Job summary

Gentiva is seeking an experienced Hospice RN / RN Case Manager to lead care for patients in the home and community settings in Mobile, AL. You will assess, plan, and deliver individualized hospice nursing services, coordinating with physicians, social workers, chaplains, and other team members to ensure compassionate, comprehensive end-of-life care.

Responsibilities include assessments, symptom management, education for patients and families, and meticulous documentation.

Qualifications

• Current license to practice nursing in the state of employment
• CPR Certification
• Valid driver's license and automobile insurance
• Minimum 1 year RN experience in acute care, hospice, home health or related field

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person or telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans with visit frequency and risk identification.
• Coordinate care across IDG and collaborate with physicians and other providers.
• Educate patients and caregivers on disease, self-care, end-of-life care, medications, nutrition.
• Document timely clinical records and communicate changes in condition to the care team.
• Support transitions across home health, palliative, and hospice levels of care.
