Job summary

Caretenders Hospice, a part of LHC Group and the Optum family, is hiring a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients in home settings. You will evaluate patient needs, coordinate care with the IDG, and implement treatment strategies to promote comfort and quality of life.

Requires current IL RN licensure, valid driver's license, and access to a reliable vehicle. CPR certification is preferred, with 1+ year clinical experience.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies patient/family needs and re-assesses every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and responses to care.
• Collaborates with patient/family, attending physician and IDG for daily care.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes on patient condition.
• Delivers care in home settings regardless of age, race, creed, or financial condition.
