Job summary

UnitedHealth Group seeks a Registered Nurse Case Manager for hospice care. You will deliver the Plan of Care, evaluate performance, and collaborate with professionals to promote physical, psychosocial, emotional, and spiritual well-being for patients and families at home.

Responsibilities include initial eligibility assessment within 48 hours, plan of care initiation, documentation, and daily collaboration with the care team. Requires RN licensure and reliable transportation; CPR preferred.

Qualifications

• Current and unrestricted RN licensure in the state of practice (LA).
• Current driver's license and access to reliable transportation.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses every 15 days.
• Initiates and coordinates the plan of care with the IDG and attending physician.
• Documents goals, interventions, and patient/family response to hospice care.
• Supervises patient/family self-care techniques and maintains accurate clinical notes.
• Collaborates with patient/family and care team to provide daily care.
