Job summary

UnitedHealth Group, part of the Optum family, seeks an RN Case Manager to coordinate compassionate hospice services. You will assess patients, partner with the interdisciplinary care team, and develop plans that meet each patient's needs and preferences.

Responsibilities include conducting assessments, teaching patients and families, documenting progress, and providing on-call support. A valid RN license, CPR, 1+ year of nursing experience, and a valid driver’s license are required.

Qualifications

• Active RN license in the required state(s)
• Current CPR certification
• At least 1 year of nursing experience
• Valid driver's license, reliable transportation, and liability insurance

Responsibilities

• Perform patient assessments and develop care plans with the care team
• Refer to other hospice disciplines or document rationale when not referring
• Prepare and maintain patient care progress records accurately and timely
• Record pain and symptom management changes and outcomes
• Educate patients, families, and caregivers on care techniques
• Communicate regularly with the clinical manager and care team
• Provide on-call coverage as assigned
• Perform other duties as assigned
