Job summary

Gentiva Hospice is seeking a compassionate Hospice RN / RN Case Manager to lead home-based nursing care for patients facing life-limiting illness. You will assess needs, develop personalized plans of care, coordinate with an interdisciplinary team, and support patients and families with education and bereavement guidance.

You will perform comprehensive assessments, deliver symptom management, and ensure accurate documentation while following regulatory standards.

Qualifications

• Must hold current license to practice nursing in the state of employment.
• Minimum of 1 year experience as an RN in acute care, oncology, long-term care, hospice or home health.
• Hospice or palliative care experience preferred; CPR certification required; valid driver’s license and auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments; provide education to patients and caregivers.
• Deliver skilled nursing interventions and symptom management per plans of care.
• Develop and update individualized plans of care and visit schedules to meet patient needs.
• Coordinate care with physicians, nurses, social workers, and other IDG members.
• Educate patients and caregivers on disease, self-care, end-of-life care, medications, nutrition, and dietary needs.
• Communicate changes in condition to the care team and document in medical records.
