Job summary

Gentiva Hospice is seeking a compassionate Registered Nurse to lead in-home hospice care as an RN Case Manager. You will assess patients, implement care plans, and coordinate with physicians and the care team to ensure comfort and dignity at end of life.

You will educate families, monitor symptoms, and document all care activities while adhering to regulatory standards and quality initiatives.

Qualifications

• Active RN license in applicable state.
• Hospice/palliative care experience preferred.
• CPR certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management.
• Coordinate care with IDG and physicians to meet goals of care.
• Educate patients and caregivers on disease processes, end-of-life planning, and medications.
• Document clinical findings and update care plans timely.
