Job summary

Gentiva Hospice is seeking an experienced Registered Nurse to serve as a Hospice RN/Case Manager, delivering compassionate, patient-centered care in patients’ homes or equivalent settings. You will perform comprehensive assessments, develop and adjust plans of care, and collaborate with physicians, social workers, chaplains, and other team members to ensure quality end-of-life care.

Qualifications

• Registered Nurse licensed in the state of employment.
• CPR certification required.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care per Plan of Care in the patient’s home or equivalent setting.
• Perform initial and ongoing assessments to monitor patient condition and adjust care.
• Collaborate with an interdisciplinary team to build personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families and educate caregivers.
